VAX-31 OPUS-1: breadth of immunity, with comparator-specific limits

October 5, 2026: pivotal immunogenicity results
OPUS-1 has reported topline results, so this is a completed data-disclosure event. Vaxcyte reported success against the prespecified primary immunogenicity framework. That framework combines comparisons across two licensed vaccines; it does not mean that every individual comparison against both products met the same margin. The distinction is particularly important for serotypes 3 and 12F versus PCV21. No approval, completed BLA or clinical disease-prevention result was announced. [1]
Capsule biology and the carrier platform
Pneumococcal capsular polysaccharides differ by serotype. Antibodies that support opsonophagocytic killing can provide a mechanistic bridge to protection, but adding antigens is useful only if functional responses to the existing serotypes are maintained. Conjugation to a protein carrier recruits T-cell help and immune memory. VAX-31 uses a cell-free, site-specific conjugation approach intended to accommodate greater antigen breadth while limiting interference among components. This is a platform rationale, not a measured clinical outcome in OPUS-1. [1] [2]
For a vaccine, the useful pharmacodynamic readout is the serotype-specific OPA response, rather than a conventional circulating drug concentration. The Phase 3 formulation delivers 3.3 micrograms per serotype except 1, 5 and 22F, which receive 4.4 micrograms. A single vaccination is followed by Month-1 immune measurements. The supplied topline material does not define a minimum antibody level that guarantees protection for an individual or quantify waning immunity beyond this early assessment.
Prior dose finding: September 2024 version
NCT06151288 was a randomized, observer-blind, PCV20-controlled study in 1,015 healthy adults aged at least 50 years: 64 in Phase 1 and 951 in Phase 2. Three VAX-31 doses were studied. Their antigen amounts were 1.1, 2.2 and 3.3 micrograms, with higher amounts for the three specified serotypes. Month-1 OPA and IgG were assessed, and safety follow-up extended six months. The release does not supply the actual randomized, vaccinated and evaluable denominator for each dose, so the total cannot be divided into fabricated equal arms. [2]
Middle and high doses met the shared-serotype noninferiority criteria for all 20 PCV20 serotypes; the low dose did so for 18. All three doses met superiority criteria for the 11 additional serotypes. These are counts of serotype tests, not counts of protected patients. A geometric mean ratio above one is also different from a statistically superior response. This earlier dataset supported dose selection but must remain separate from the much larger OPUS-1 dataset and its PCV21 comparisons.

OPUS-1 population and testing hierarchy
NCT07284654 is randomized, double-blind and active-controlled at 30 U.S. sites. Of 4,047 dosed participants, 3,572 aged at least 50 were allocated 1:1:1 to VAX-31, PCV20 or PCV21; 475 aged 18–49 were allocated 3:1 to VAX-31 or PCV20. These allocation ratios do not establish actual arm counts. Vaccination occurred on Day 1, immune assessment at Month 1, solicited reactions during Days 1–7 and safety follow-up through six months. Detailed exclusions, per-protocol exclusions, missing-titer handling and the full estimand were not supplied in the retrieved topline release. [1]
The shared-serotype primary rule used a lower confidence bound above 0.667 for OPA geometric mean ratios. For 11 serotypes shared with both comparators, success versus one or both was assessed with a Hochberg adjustment at one-sided adjusted p<0.025. Nine serotypes were unique to the PCV20 comparison and eight to PCV21. Superiority for three VAX-31-specific serotypes and cross-reactive 20B required a lower bound above 2.0. These different tests cannot be collapsed into a single efficacy percentage.
Results: preserve the comparator distinction
All 28 shared primary comparisons met the combined framework. Individually, VAX-31 met the 0.667 margin for 20/20 PCV20 serotypes and 17/19 PCV21 serotypes. Serotypes 3 and 12F versus PCV21 met a less stringent historical 0.5 secondary margin, not the 0.667 margin. Younger-to-older immunobridging met noninferiority for 32 measured responses, comprising the vaccine serotypes plus cross-reactivity; this does not make the product a 32-valent vaccine. [1]
Analysis: the data support broad functional immunogenicity and advancement of the adult program. They do not directly quantify reductions in pneumonia, hospitalization or invasive disease. Exact serotype GMRs and confidence intervals were not extractable from the retrieved announcement and are not reconstructed from pass/fail counts. Differences in serotype prevalence, immune persistence and the eventual approved population will affect how this immune bridge translates into clinical protection.

Safety and unreported denominators
The sponsor described a safety profile comparable to the licensed controls, with solicited reactions predominantly mild or moderate and usually resolving within 48 hours. No vaccine-related serious adverse events or adverse-event discontinuations were reported. Neither statement means that no serious events or deaths occurred from any cause. Without arm-level event counts, total safety denominators and severity distributions, a numerical tolerability advantage cannot be established. Older and younger cohorts also should not be pooled without accounting for their different comparator allocation. [1] [2]

Program execution and next evidence
OPUS-2 and OPUS-3 results remain guided for the first half of 2027, addressing concomitant influenza vaccination and previously vaccinated adults. The company anticipates an adult BLA in the first half of 2028, contingent on the remaining package. The infant program has a separate first-half-2027 guidance window and cannot borrow adult evidence as an infant efficacy conclusion. These forecasts are not fixed dates or completed milestones. [1] [3] [4]
On October 5 management also proposed approximately $500 million of common stock/pre-funded warrants and $500 million of convertible notes. Proposed gross proceeds are not cash already received; convertible debt adds obligations and warrants represent potential equity issuance. For trial execution, the relevant questions are whether manufacturing, other OPUS studies and longer follow-up can be funded and delivered. The financing announcement provides context for those activities, not evidence that the clinical or regulatory milestones will succeed.
